UC-II (undenatured collagen type II)
Distinct 40 mg/day undenatured collagen with knee-OA RCT signal: different mechanism from hydrolysed collagen peptides.
Why
Undenatured collagen type II (UC-II) is a low-dose (40 mg/day) collagen preparation distinct from the higher-dose hydrolysed collagen peptides: same source molecule, different processing, different mechanism. The Lugo 2016 RCT (n=191 moderate knee OA) reported UC-II 40 mg/day produced larger WOMAC pain and function improvements than glucosamine + chondroitin 1,500 + 1,200 mg/day over 180 days. Effect is via oral immunological tolerance, not structural collagen replacement.
How it works
The 40 mg dose passes through Peyer's patches in the small intestine, inducing oral tolerance that downregulates the autoimmune-like inflammatory response against joint collagen type II: proposed key mechanism distinct from substrate provision in hydrolysed collagen.
Expected onset · Symptomatic effect over 8–12 weeks; trial endpoints at 180 days
How to take
Dosage
40 mg/day single dose. Higher doses do not produce greater benefit; the oral tolerance mechanism is dose-specific.
Timing
Once daily, on empty stomach (to favour Peyer's patch interaction)
On the label
'UC-II' (trademarked InterHealth/Lonza ingredient) at 40 mg/day. Standard hydrolysed collagen peptides at higher doses are a different supplement with separate evidence (covered in `collagen-peptides`).
Ideal for
Adults with knee OA seeking an alternative to glucosamine + chondroitin; people who prefer a low-dose once-daily option to high-dose hydrolysed collagen.
Safety
Evidence
Lugo 2016 Nutr J head-to-head RCT (n=191 moderate knee OA): UC-II 40 mg/day improved WOMAC pain by ~33% vs ~14% for glucosamine + chondroitin over 180 days. Crowley 2009 90-day RCT confirmed superior symptom improvement vs the glucosamine + chondroitin combination. Distinct mechanism and lower dose vs hydrolysed collagen peptides.
