Calendula topical

EMA-monograph topical herb for minor wounds and skin inflammation, single positive RCT in radiation dermatitis vs trolamine.

Why

Calendula officinalis flower preparations hold an EMA HMPC monograph for topical use in minor wounds, skin inflammation, and mucosal complaints. A single sizeable RCT in breast-cancer radiation dermatitis (Pommier J Clin Oncol 2004) found calendula significantly outperformed trolamine for prevention of grade ≥2 dermatitis, that trial remains the strongest single dermatology data point. Smaller RCT bodies support effect on minor abrasions, nappy rash, and chronic venous leg ulcers as part of compression regimens.

How it works

Triterpene saponins (faradiol esters) and flavonoids stimulate fibroblast proliferation and collagen deposition. Carotenoids and polysaccharides reduce local oxidative stress and support keratinocyte migration during re-epithelialisation.

Expected onset · Minor skin healing within days; radiation dermatitis prevention assessed across the radiotherapy course

How to take

Dosage

Topical cream, ointment, or hydrogel: apply 2–3 times daily to the affected area. For radiation dermatitis: from the start of radiotherapy through completion.

Timing

2–3 times daily during the symptomatic or at-risk phase

On the label

Standardised flower extract in a cream or hydrogel base, concentration of marigold tincture in the finished product matters more than total weight.

Ideal for

Adults at risk of radiation dermatitis during breast-cancer radiotherapy; minor abrasions, post-procedural skin healing, nappy rash.

Safety

Allergic contact dermatitis possible, particularly in those sensitive to Asteraceae (ragweed, daisies). Apply to clean skin; not for deep or infected wounds without medical guidance. Pregnancy: topical use considered low risk; internal preparations are not recommended in pregnancy.

Evidence

At a glance

Pommier 2004 J Clin Oncol RCT (n=254 breast-cancer radiotherapy): topical calendula cream significantly reduced grade ≥2 acute dermatitis vs trolamine (41% vs 63%). EMA classifies the flower as Traditional Use for minor wounds and skin inflammation. Givol 2019 systematic review confirmed directional benefit across diverse wound types.
