Witch hazel (Hamamelis virginiana)

EMA-monograph topical astringent for skin inflammation, minor wounds, and haemorrhoidal symptoms.

Why

Witch hazel (Hamamelis virginiana) bark and leaf preparations hold EMA HMPC monographs at Well-Established Use for symptomatic relief of skin inflammation, bruises and discomfort due to haemorrhoids. The aqueous distillate (hamamelis water) is the most-used topical form. Astringent and anti-inflammatory effects support use in eczema, contact dermatitis, anal pruritus, and minor abrasions.

How it works

Tannins (hamamelitannin) precipitate surface proteins, producing astringent (tightening) effect and reducing inflammatory exudation. Anti-inflammatory effects on cyclooxygenase and leukotriene pathways. Mild vasoconstrictor effect on superficial vessels, relevant for haemorrhoid symptoms.

Expected onset · Symptomatic relief within hours; full effect over days

How to take

Dosage

Topical hamamelis water or ointment (10–25% extract): apply 2–4 times daily to affected area. Suppositories for internal haemorrhoids: per product label.

Timing

2–4 times daily during symptomatic phase

On the label

Hamamelis water (aqueous distillate) for gentle topical use; ethanolic tincture for stronger astringent effect. Stated tannin content if pharmacopoeial.

Ideal for

Adults with mild eczema flares, contact dermatitis, anal pruritus, haemorrhoidal discomfort, or minor skin abrasions.

Safety

Topical use. Internal use is not the indication (tannins can cause GI upset). Apply to intact skin; not for deep, infected wounds. Allergic contact dermatitis possible. Some preparations contain alcohol, can sting on broken skin. Pregnancy: topical use considered low-risk for limited areas.

Evidence

At a glance

EMA classifies hamamelis preparations at Well-Established Use for skin inflammation and haemorrhoidal symptoms. Korting 1995 controlled trial confirmed anti-inflammatory effect of hamamelis distillate on UV-erythema and chemically-induced irritation. Modern dermatology evidence is small but the regulatory anchor is high-tier.
