St. John's wort oil (topical)

EMA-monograph red oil maceration for minor wounds, mild sunburn, and muscle-related skin discomfort. Separate from the antidepressant oral use.

Why

St. John's wort macerated oil (the red infused oil of Hypericum perforatum flowering tops in olive or sunflower oil) holds an EMA HMPC monograph at Traditional Use for symptomatic treatment of minor inflammations of the skin (such as sunburn) and as an aid in the healing of minor wounds. The active hyperforin and hypericin are stable in oil maceration. Distinct from the oral antidepressant extract, same plant, different preparation and indication.

How it works

Hyperforin and hypericin in the oil maceration produce anti-inflammatory effects on cyclooxygenase pathways and stimulate fibroblast proliferation. The red colour comes from hypericin photodimers and is a visual marker of correct preparation.

How to take

Dosage

Topical oil or ointment: apply 2–3 times daily to affected area for up to 2 weeks.

On the label

Macerated oil of Hypericum perforatum flowering tops, should be deep red colour. Olive oil or sunflower oil base. Distinct from St John's wort tincture or dry extract (for oral antidepressant use).

Ideal for

Adults with minor abrasions, mild sunburn, post-procedural skin healing, or muscle-related skin tenderness seeking a traditional topical preparation.

Safety

Hypericin causes photosensitisation. Avoid sun exposure to treated skin for 12–24 hours after application. Topical use only. Internal use is a separate indication with a substantial drug-interaction profile (see oral St John's wort card). Apply to intact skin; not for deep or infected wounds. Allergic skin reactions possible. Pregnancy and breastfeeding: topical use considered low-risk but data limited.

EMA classifies the macerated oil at Traditional Use for minor skin inflammations and minor wound healing. Samadi 2010 RCT (n=144 caesarean-section women): Hypericum oil-treated wounds had significantly better healing scores and lower pain than placebo or routine care at 10 days.
