Saw palmetto
EMA Well-Established Use for BPH symptoms, Cochrane evidence is mixed but the standardised hexane extract (Permixon) carries the cleanest positive signal.
Why
Saw palmetto (Serenoa repens) berry extract holds an EMA HMPC monograph at the Well-Established Use level for relief of lower urinary tract symptoms related to benign prostatic hyperplasia. The Cochrane review's overall conclusion was that saw palmetto is not more effective than placebo for lower urinary tract symptoms, but the hexanic lipidosterolic extract (Permixon), distinct from many off-the-shelf products, has shown effects in head-to-head trials comparable to tamsulosin and finasteride. Product form is the central distinction.
How it works
Inhibits 5α-reductase (the enzyme that converts testosterone to dihydrotestosterone), modulates androgen receptor signalling, and has anti-inflammatory effects in prostatic tissue. The hexanic extract has distinct phytosterol profile vs ethanolic and CO2 extracts, likely accounting for the differential trial results.

Dosage
Hexanic extract (Permixon): 320 mg/day single dose. Standard liposterolic extract: 160 mg twice daily.

'Hexanic lipidosterolic extract' or 'Permixon' carries the strongest evidence. Generic ethanolic or CO2 extracts have inconsistent trial results.
Ideal for
Adult men with mild-to-moderate lower urinary tract symptoms attributable to BPH, after specialist evaluation has excluded prostate cancer.
Safety
Evidence
Cochrane 2012 SR (32 RCTs, n=5,666) concluded saw palmetto extracts overall not better than placebo for BPH symptoms. Vela-Navarrete 2018 BJU Int meta-analysis of the hexanic Permixon extract specifically (15 RCTs, n=3,053): significant IPSS reductions vs placebo and non-inferiority to tamsulosin and short-cycle finasteride. EMA classifies the extract as Well-Established Use, the higher regulatory tier.
