Sage (Salvia officinalis)

EMA-monograph herb for sore throat (gargle) and excessive perspiration including menopausal hot flushes.

Why

Sage (Salvia officinalis) holds EMA HMPC monographs for two indications: relief of mild inflammations of the mouth and throat (Well-Established Use, topical gargle/spray) and symptomatic relief of excessive perspiration including night sweats. RCTs in menopausal vasomotor symptoms report reductions in hot flush frequency and severity.

How it works

Tannins, rosmarinic acid and essential oil components have local astringent and antiseptic effect in the oropharynx, the basis of the throat indication. Anti-cholinergic activity at sweat-gland muscarinic receptors and central serotonergic effects contribute to the antihidrotic and menopausal signal.

Expected onset · Sore throat: within hours. Hot flushes: typically over 4–8 weeks.

How to take

Dosage

Sore throat: gargle/spray three times daily with a standardised preparation (e.g. tincture diluted in water). Hot flushes: 300 mg dried-leaf tablet daily, or a standardised fresh-leaf extract per product label.

Timing

Sore throat: at symptom onset, repeat 3–4× daily. Hot flushes: daily, morning or split dosing.

On the label

For sore throat: aqueous extract or essential-oil spray with stated rosmarinic acid content. For hot flushes: dried-leaf tablets or fresh-leaf preparations, not the pure essential oil.

Ideal for

Adults with sore throat (gargle/spray formulations); menopausal women with troublesome hot flushes seeking a non-hormonal botanical option.

Safety

Sage essential oil contains thujone. Avoid prolonged internal use of essential oil or high-dose extracts (thujone is neurotoxic at high doses). Aqueous and standardised leaf preparations are lower-risk. Avoid in pregnancy and breastfeeding (uterotonic at internal doses; may reduce milk supply). Caution in epilepsy and oestrogen-sensitive cancers (theoretical oestrogenic effect).

Hubbert 2006 RCT (n=286): a 15% sage spray significantly reduced throat pain vs placebo at 2 hours. Bommer 2011 open trial in menopausal hot flushes: 50% reduction in flush frequency over 8 weeks.
