Red clover (isoflavones)

Phytoestrogen source with moderate signal for hot flushes, effect smaller than black cohosh but with a different mechanism.

Why

Red clover (Trifolium pratense) is a rich source of isoflavone phytoestrogens (biochanin A, formononetin, daidzein, genistein). Meta-analyses of RCTs in postmenopausal women report a modest reduction in daily hot flush frequency versus placebo. Effect is smaller than oestrogen-based options and smaller than black cohosh in head-to-head comparisons, but offers an alternative when other options are declined or not tolerated.

How it works

Isoflavones bind weakly to oestrogen receptor β (selective oestrogen receptor modulator-like effect), more strongly than the soy isoflavones at clinical doses. Effect on the thermoregulatory threshold reduces hot flush frequency in some women.

Dosage

Standardised isoflavone extract: 40–80 mg/day total isoflavones (Promensil, Menoflavon).

Standardised to total isoflavone content (e.g. Promensil 40 mg, Menoflavon 80 mg). Raw red clover tea is too dilute for the trial doses.

Ideal for

Postmenopausal women with troublesome hot flushes who prefer a phytoestrogen approach over central-acting botanicals.

Safety

Avoid in oestrogen-sensitive malignancy (breast, endometrial, ovarian), phytoestrogen activity is real even if weak. Caution with tamoxifen, aromatase inhibitors, and combined hormonal contraceptives. Possible mild antiplatelet effect via the coumarin content. Caution with anticoagulants. Pregnancy and breastfeeding: avoid.

Myers 2017 Maturitas meta-analysis (5 trials with daily flush frequency data): red clover isoflavones reduced daily flushes by 1.7 vs 0.8 on placebo. Cochrane 2013 SR concluded a modest signal at higher isoflavone doses (≥80 mg/day genistein-equivalent). Effect smaller than HRT and smaller than black cohosh in indirect comparison.
