Korean ginseng

EMA monograph covers traditional use for fatigue.

Why

EMA monograph covers traditional use for fatigue. Multiple RCTs support cognitive and physical performance in cancer-related and idiopathic chronic fatigue.

How it works

Ginsenosides modulate HPA-axis and neurotransmitter signalling; EMA HMPC recognises traditional use for symptoms of fatigue and asthenia.

Expected onset · Subjective fatigue effects often emerge by 4–8 weeks

How to take

Dosage

Powdered root: 600–2000 mg/day in divided doses (EMA). Standardised extract (e.g. G115): 200–400 mg/day. Cancer-related fatigue trials have used 1000–2000 mg/day.

Timing

Morning and early afternoon; avoid evening (can be stimulating)

Safety

Contraindicated in pregnancy and breastfeeding (EMA). May lower blood glucose (caution with antidiabetics), increase blood pressure, and interact with warfarin, MAOIs, and stimulants. Consult a doctor if symptoms persist beyond 2 weeks.

Evidence

At a glance

EMA HMPC monograph registers Panax ginseng root as a Traditional Herbal Medicinal Product for symptoms of fatigue and weakness: traditional-use registration based on long medical use. Modern RCT evidence is strongest in cancer-related fatigue (Mayo Clinic and others have published positive trials in oncology populations); idiopathic-fatigue evidence is more modest.
