Cartilage building blocks with patchy but real RCT signal for moderate-to-severe knee osteoarthritis pain.
Why
Glucosamine sulfate and chondroitin sulfate are endogenous components of joint cartilage. The evidence has been notoriously inconsistent: early industry-sponsored trials were positive, the large independent GAIT trial was null overall, and subgroup analysis suggested benefit only in moderate-to-severe knee pain. The MOVES head-to-head trial later showed non-inferiority of the combination vs celecoxib over 6 months.
How it works
Substrate provision for glycosaminoglycan synthesis in articular cartilage; modest anti-inflammatory effects on chondrocyte NF-κB signalling. Chondroitin may also inhibit cartilage-degrading enzymes (MMP-3, MMP-13).

Dosage
Glucosamine sulfate 1,500 mg/day (preferably as a single daily dose, the EU-licensed form) + chondroitin sulfate 800–1,200 mg/day.

Evidence
Clegg 2006 NEJM GAIT trial (n=1,583): overall null result for the full cohort, but the prespecified moderate-to-severe pain subgroup showed significant benefit on the combination vs placebo. Hochberg 2016 Ann Rheum Dis MOVES (n=606): glucosamine sulfate + chondroitin sulfate non-inferior to celecoxib for WOMAC pain reduction over 6 months. Effect size modest; product quality matters.
