Why
Boswellia serrata (Indian frankincense) gum resin has been used in Ayurveda for inflammatory joint complaints for centuries. RCTs of standardised extracts (5-Loxin, Aflapin, AKBA-enriched) in symptomatic knee osteoarthritis report reductions in pain, stiffness and functional limitation versus placebo over 30–90 days. Meta-analyses are limited but directionally consistent.
How it works
Boswellic acids, particularly acetyl-11-keto-β-boswellic acid (AKBA), selectively inhibit 5-lipoxygenase, reducing leukotriene synthesis. Distinct from NSAID mechanism; no inhibition of prostaglandins or platelet function.

Yu 2020 BMC Complement Med Ther meta-analysis (7 RCTs, n=545): Boswellia serrata significantly reduced WOMAC pain, stiffness and function scores vs placebo over 4–24 weeks. Sengupta 2008 5-Loxin RCT (n=75): 100 mg and 250 mg/day of AKBA-enriched extract significantly improved knee OA pain by 7 days, sustained at 90 days. Effect modest, side-effect profile favourable.
