Arnica topical
EMA-monograph topical arnica for bruising and hand osteoarthritis, head-to-head non-inferiority to ibuprofen gel.
Why
Arnica (Arnica montana) flower-head preparations hold an EMA HMPC monograph as a Traditional Herbal Medicinal Product for topical use in bruises, sprains, and rheumatic complaints. The Widrig RCT directly compared topical arnica gel with ibuprofen gel for hand osteoarthritis and reported non-inferior pain reduction and function improvement over 3 weeks. Other RCTs report reduced bruising after laser surgery and improved post-surgical recovery.
How it works
Sesquiterpene lactones (helenalin, dihydrohelenalin) inhibit NF-κB activation, reducing pro-inflammatory cytokine production at the site of application. Flavonoids contribute additional antioxidant effects on the inflamed tissue.
Expected onset · Symptom relief often within days for bruising; 1–2 weeks for hand OA

Safety
Evidence
Widrig 2007 Rheumatol Int RCT (n=204 hand OA): topical arnica gel was non-inferior to ibuprofen 5% gel for hand pain and function over 3 weeks, with similar adverse-event rates. EMA classifies topical arnica preparations as Traditional Use for bruising, sprains and rheumatic complaints. Iannitti 2016 Adv Ther review summarises positive signal across post-surgical and trauma indications.
