Fermented apple vinegar with meta-analysis signal for modest postprandial glucose blunting and small effects on fasting glucose and lipids.

Why

Apple cider vinegar (ACV), fermented apple cider containing 4–7% acetic acid, has become one of the most-asked-about kitchen-cabinet remedies. The Hadi 2021 systematic review and meta-analysis (9 RCTs) confirmed modest reductions in fasting glucose, HbA1c, total cholesterol and LDL versus control. Effects are smaller than pharmaceutical alternatives but real and dietary-friendly.

How it works

Acetic acid delays gastric emptying, slowing postprandial glucose absorption. Inhibits hepatic gluconeogenesis and disaccharidase activity. Modulates AMPK signalling in skeletal muscle.

How to take

Dosage

15–30 ml (1–2 tablespoons) of 5% apple cider vinegar, diluted in 200 ml of water, with meals. Trials use 15–30 ml/day.

With or just before main meals

On the label

Raw, unfiltered ACV 'with the mother' is most popularly used (no clear evidence of superiority); pasteurised is acceptable. Capsule forms are less acidic but also less studied.

Ideal for

Adults with prediabetes, postprandial glucose elevations, or mild dyslipidaemia exploring food-based adjuncts.

Safety

Always dilute. Undiluted ACV (pH ~2–3) causes dental enamel erosion, oesophagitis, and case reports of oesophageal injury. Drink through a straw and rinse mouth after. Acid reflux: can worsen. Drug interactions: hypokalaemia at very high chronic doses; caution with digoxin and diuretics. Pregnancy: culinary doses fine.

Hadi 2021 BMC Complement Med Ther meta-analysis (9 RCTs, n=684): ACV reduced fasting glucose by 0.50 mmol/L, HbA1c by 0.39%, total cholesterol by 0.30 mmol/L and LDL by 0.32 mmol/L vs control.
