Andrographis paniculata

Cochrane-supported 'king of bitters' for upper respiratory infection symptom relief.

Why

Andrographis paniculata is a bitter herb widely used in Indian, Chinese, and Scandinavian phytotherapy. The Cochrane review of Andrographis for acute respiratory tract infection found that the standardised extract Kan Jang (containing andrographolides plus Eleutherococcus) shortened symptom duration and reduced overall symptom severity compared with placebo. Effect is most consistent for cough, sore throat, and rhinorrhoea.

How it works

Andrographolides modulate cytokine signalling (TNF-α, IL-6), inhibit NF-κB, and show direct antiviral activity against influenza A in vitro. Mucosal immune effects are documented in animal models.

Dosage

Standardised extract delivering 60–120 mg/day total andrographolides (Kan Jang dose: 1 tablet three times daily for 5 days from symptom onset).

Safety

Common GI side effects (loss of appetite, nausea, diarrhoea). Bitter taste can worsen reflux. Possible additive effects with antihypertensives and anticoagulants. Avoid in pregnancy (animal studies show reproductive toxicity). Allergic reactions, including anaphylaxis, are rare but reported. Limit use to acute episodes (1–2 weeks) rather than chronic use.

Hu 2017 PLoS One meta-analysis (33 RCTs, n=7,175): Andrographis significantly reduced cough and sore throat severity in upper respiratory tract infection vs placebo and standard care. Coon 2004 earlier systematic review reached the same conclusion. Used acutely from symptom onset, not chronically.
